These Terms & Conditions govern your use of our website, online booking features, forms, communications, and mobile phlebotomy and specimen collection services.

 

By accessing this website, scheduling an appointment, submitting a service request, or using our services, you acknowledge and agree to these

 

Terms & Conditions.

 

1. OUR SERVICES

 

The Mobile Phlebotomist provides professional mobile blood collection and specimen collection services throughout Greater New Orleans and surrounding communities.

Services may include:

• Routine blood draws
• Specialty testing kit collections
• STAT collections
• Gender reveal blood draws
• Specimen preparation and handling
• Mobile collection services for healthcare providers, facilities, employers, and organizations
• Other related collection services as available

 

Services are subject to availability, location, scheduling, testing requirements, laboratory requirements, and other applicable conditions.

 

2. MOBILE PHLEBOTOMY SERVICES

 

The Mobile Phlebotomist provides specimen collection services and does not replace your physician, laboratory, or other healthcare provider.

 

Unless otherwise specifically stated, we do not diagnose medical conditions, prescribe medications, interpret laboratory results, recommend medical treatment, or provide medical advice through this website.

 

Questions concerning your health, laboratory results, medical treatment, medications, fasting requirements, or the medical necessity of a particular test should be directed to your healthcare provider or the appropriate laboratory.

 

3. LAB ORDERS AND REQUISITIONS

 

Certain laboratory collections require a valid laboratory order, requisition, testing kit, or other documentation.

 

Patients are responsible for having all required documentation and testing materials available before or at the time of their appointment.

 

Some eligible testing may be available through third-party direct-access testing providers without a traditional physician's order. Requirements vary according to the test, laboratory, provider, and applicable laws or regulations.

 

The Mobile Phlebotomist reserves the right to decline or reschedule a collection when required documentation, testing materials, instructions, or other necessary information is unavailable or incomplete.

 

4. ONLINE BOOKING

 

Our website allows customers to schedule eligible mobile collection services online.

 

When scheduling an appointment, you are responsible for providing complete and accurate information, including:

• Your name and contact information
• Appointment location
• Requested service
• Requested appointment date and time
• Any other information reasonably necessary to coordinate your collection

 

Submitting or completing an online booking does not guarantee that a collection can be performed if the necessary laboratory requirements, documentation, collection materials, or other conditions are not met.

 

We may contact you before your appointment if additional information or coordination is required.

 

5. SERVICE AREA AND MILEAGE FEES

 

The Mobile Phlebotomist primarily serves Greater New Orleans and surrounding communities.

 

If your appointment location is more than 25 miles from New Orleans, Louisiana, please contact us before booking.

 

An additional mileage fee applies to locations outside the 25-mile service radius.

 

Mileage fees are based on the total round-trip mileage required to provide the service and will be discussed with you before the appointment is confirmed.

 

We reserve the right to decline service to locations outside our available service area.

 

6. SERVICE PRICING

 

Current mobile collection fees may include:

Routine Blood Draw — $65
Specialty Testing Kit Collection — $85
STAT Collection — $85
Gender Reveal Blood Draw — $100

 

Prices are subject to change.

 

Additional charges may apply depending on travel distance, collection requirements, specialty testing requirements, additional services, or other circumstances communicated before service when reasonably possible.

 

Unless expressly stated otherwise, mobile collection fees are separate from laboratory testing, processing, shipping, physician, or third-party testing fees.

 

7. PAYMENT

 

Payment may be required when booking, before the appointment, or at the time services are provided.

 

Customers are responsible for all applicable charges associated with their requested mobile collection service, including any approved travel or additional service fees.

 

Laboratory testing and processing charges are separate from The Mobile Phlebotomist's mobile collection fees unless expressly stated otherwise.

 

Laboratories and third-party testing providers maintain their own pricing, insurance, billing, refund, and payment policies.

 

8. INSURANCE

 

The Mobile Phlebotomist's mobile convenience and collection fees are generally paid directly by the patient unless another arrangement has been established.

 

Laboratory processing may be billed separately by the laboratory and may be eligible for insurance coverage depending on the laboratory, test, insurance provider, and individual plan.

 

The Mobile Phlebotomist does not guarantee insurance coverage, reimbursement, or payment by an insurance company.

 

Patients should contact their insurance provider or testing laboratory with questions concerning laboratory coverage and benefits.

 

9. APPOINTMENT PREPARATION

 

You are responsible for following any preparation instructions provided by your healthcare provider, laboratory, or testing company.

 

This may include requirements concerning fasting, hydration, medications, timing, testing kits, identification, or other preparation.

 

The Mobile Phlebotomist does not independently determine whether you should fast, discontinue medication, or otherwise modify medical treatment.

 

Contact your healthcare provider or laboratory if you are unsure how to prepare for your collection.

 

10. APPOINTMENT AVAILABILITY

 

Appointment availability may vary based on location, scheduling, staffing, service type, travel requirements, and other operational factors.

 

Same-day appointments may be available but are not guaranteed.

 

STAT collection refers to priority mobile collection and does not guarantee laboratory processing times or when results will become available.

 

11. SPECIMEN HANDLING AND LABORATORY PROCESSING

 

The Mobile Phlebotomist is responsible for providing the agreed collection service and handling specimens according to applicable collection requirements.

 

Specimens may be transported or otherwise coordinated with the appropriate designated laboratory based on the applicable testing requirements.

 

Once a specimen has been transferred to a laboratory or other authorized third party, laboratory processing, testing, result reporting, and turnaround times are controlled by that organization.

 

The Mobile Phlebotomist cannot guarantee laboratory processing times or the timing, accuracy, or availability of laboratory results.

 

12. LABORATORY RESULTS

 

The Mobile Phlebotomist generally does not interpret laboratory results or provide medical diagnoses based on laboratory testing.

Results are provided according to the procedures of the laboratory or testing provider processing the specimen.

 

For physician-ordered testing, results may be sent directly to the ordering healthcare provider.

 

Questions regarding results, abnormal findings, diagnosis, or treatment should be directed to the appropriate healthcare provider or laboratory.

 

13. THIRD-PARTY TESTING SERVICES

 

Our website may provide information or links relating to third-party testing services, including Rupa Health and Ulta Lab Tests.

 

Testing purchased or ordered through third-party providers is subject to that provider's own terms, eligibility requirements, pricing, availability, privacy practices, refund policies, and laboratory procedures.

 

The Mobile Phlebotomist's collection fee is separate from third-party laboratory or testing fees unless specifically stated otherwise.

 

14. PROFESSIONAL AND FACILITY SERVICES

 

Healthcare providers, facilities, employers, clinical programs, and other organizations may contact us regarding one-time, recurring, group, or multi-patient collection services.

 

Submitting a professional services inquiry does not establish a contract or guarantee service availability.

 

Service requirements, scheduling, pricing, collection volume, locations, and other terms may be established separately before professional services begin.

 

15. CANCELLATIONS, RESCHEDULING, AND REFUNDS

 

Appointments may be subject to cancellation, rescheduling, late-cancellation, no-show, or refund requirements.

 

Any applicable cancellation or refund policy communicated during booking or published separately on our website will apply to your appointment.

 

If you need to cancel or reschedule, please contact The Mobile Phlebotomist as soon as possible.

 

Additional charges may apply when travel has already begun, a phlebotomist has arrived at the appointment location, or circumstances outside our

control prevent the collection after arrival, subject to the applicable cancellation and refund policy.

 

16. UNSUCCESSFUL OR INCOMPLETE COLLECTIONS

 

Although every reasonable effort will be made to complete the requested collection, successful specimen collection cannot be guaranteed in every circumstance.

 

A collection may need to be discontinued, rescheduled, or referred elsewhere due to patient safety, collection difficulty, inadequate testing materials, missing documentation, specimen requirements, or other circumstances.

 

Any fees applicable to an unsuccessful or incomplete collection will be handled according to the applicable booking, cancellation, or refund policy.

 

17. PATIENT SAFETY

 

Patient safety is a priority.

 

The Mobile Phlebotomist reserves the right to decline, discontinue, or reschedule a collection when proceeding would be inappropriate or unsafe.

 

Mobile phlebotomy services are not emergency medical services.

 

If you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.

 

18. WEBSITE INFORMATION

 

We make reasonable efforts to keep information on this website accurate and current. However, services, prices, availability, service areas, policies, and other information may change.

 

Website content is provided for general informational purposes and should not be considered medical advice.
